ELECTRONIC COMMUNICATIONS CONSENT

Flourish Psychology prefers to send all information and communications electronically. You consent to receive all communications, information, notices and documents by electronic communication. This consent constitutes consent to give and receive information electronically pursuant to the Electronic Transactions (Queensland) Act 2001 (Qld). You are authorised to provide this consent if you are a client/patient or the parent or caregiver of a child or children who are clients/patients of Flourish Psychology.

1. Electronic Communication

Electronic communication means communication:

(a) Sent to an email address nominated by you;

(b) Sent to you via text message or SMS; or

2. Terms and Conditions

2.1 Flourish Psychology will not also issue a paper copy of information, communications, notices or documents unless it chooses to do so or at your request.

2.2 You may remove your consent to receive electronic communication at any time by written notice to Flourish Psychology.

2.3 It is your responsibility to:

(a) Operate and maintain appropriate equipment and software to access, complete, view, process, retrieve, save, and print electronic communications from Flourish Psychology;

(b) Ensure your nominated mobile number and email address are capable of receiving electronic communications from Flourish Psychology;

(d) Maintain and/or keep a record of electronic communications from Flourish Psychology;

(e) Maintain confidentiality in all electronic communications from Flourish Psychology; and

(f) Notify us if your mobile number or email address change or if you wish to withdraw/terminate this consent.

3. Receipt

Any communication or information sent by Flourish Psychology to you will be deemed to have been received by you at the time the communication enters your information system or is capable of being viewed, irrespective of whether or not you aware of its receipt.

4. Health information

This consent covers the electronic exchange of health information between you and our health professionals.

5. Access

You may request a paper copy of any notice, communication or document given to you by Flourish Psychology electronically within 7 years of the electronic communication being received by you.
